What to bring to your first Evolve Physical Therapy Appointment:

• Proper forms of identification

• Insurance card(s)

• Payment information

• Doctor’s referral

• New Patient Forms

• Appropriate attire so that the physical therapist may assess the area of injury

What to expect during your initial evaluation and physical therapy sessions:

• Please allow for a minimum of 60 minutes for the initial evaluation.

• Each follow-up session will last 45-60 minutes, depending on the treatment required that day.

• Please arrive 15 minutes early if you are unable to fill out your new patient forms prior to your appointment.
